It used in obstetric surgery to detect amniotic fluid leaks. It has been used in chromoendoscopy as an aid to visualize the mucosa. In urological surgery, it is intravenously injected into the body to highlight the urinary tract. It immediately colours the urine as it is easily filtered by the kidneys, thus enabling the detection of any leak in the urinary tract. But indigo carmine has the potential to dangerously increase the blood pressure in some cases. In the pharmaceuticals industry it is used as a dye in the manufacturing of capsules.

It is also used as a pH indicator in higher ranges of ph. It is based on the redox mechanism turning yellow at pH above 13 and blue at 11.4.
